Absolute Beginner’s guide to Immunotherapy treatments.



If you’re looking for an introduction to immunotherapy and its different applications, this blog-post will save you a lot of headache and will show the different applications of this type of treatment in layman language.

We’ll be introduced as to what is Immunotherapy, its different types and uses, and we’ll go through with only two types as a start.

First of all what is immunotherapy?
It is the treatment of any type of disease, through the enhancement,suppression, or induction of a response from the immune system.

Now that we know what it’s about, let’s get to understand its different types as follows:
There are two main types: Activation & Suppression Immunotherapies, and other approaches which will introduce in other posts.

We’ll get to exploring them more in detail in other articlesbut before we’re capable of understanding them we have to understand what are immune modulators.

Immuno-modulators are mainly the active agents of this type of treatment, and they’re made of various preparations, that could be natural, synthetic or even recombinant, Cytokines are often the case, but let’s take a look at several examples of them:

1-Cytokines:Interferons, G-CSF
2-ChemoKines:CCL3, CCL26
3-Interleukins: IL2 – IL-12
4-others: cytosine phosphate guano sine,glucans.

What makes these type of active agents attractive for usage as a regimen is that they often have side effects that are not as strong or unfavorable as drugs and also create less resistance when it comes to microbial diseases.

The main type of treatment these immunomodulators are used for is called cell based immunotherapies , which has proven to be effective for some kind of cancer , Immune effector cells are like natural killer cells , dendritic cells ,  macrophages , cytotoxic t lymphocytes …etc. .
Mainly the way to work to protect the body is by targeting abnormal antigen on the surface of the cancerous cells that have mutated.

Immunotherapy rises as new approach toward breast cancer treatment.

Breast cancer is found as the second most common cancer in women in the United States, while it is one of the leading causes of cancer death across the world. In Lobular Carcinoma in situ (LCIS), cancer is arises in the lobule of the breast. LCIS is most common cancer, in the case of pre-menopausal in women.

Breast cancer cells cross the lining of the lobule or milk duct, and begin to invade adjacent tissues, it is known as infiltrating cancer. Breast cancer is considered infiltrating or invasive, when the cancer cells have penetrated the membrane that surrounds a duct or lobule. This type of cancer forms a lump that can eventually be felt by a physical examination.

 BCG is a bacterium which is associated to the germ and it causes tuberculosis, but it does not generally cause serious types of diseases.

What exactly is BCG used for?
BCG is indicated for the prophylaxis and treatment of carcinoma in situ of urinary bladder cancer, and used for the prophylaxis of primary stage Ta or T1 papillary tumors following transurethral resection (TUR). BCG is not suggested for the stage TaG1 papillary tumors, if they are concluded to be at high risk of tumor repetition.
Although it’s advised not to be used in immunosuppressed patients or patients with acquired or congenital immune deficiencies, whether due to immunosuppressive therapy, cancer therapy or concurrent disease such as AIDS.

Is there are any noted side-effects?
Yes, the most common side effects include: Hematuria, flu-like syndrome, urinary frequency, dysuria, malaise/fatigue, fever, nausea/vomiting, rigors, cramps/pain, nocturia, urgency, and cystitis.

Immunotherapies make a strong immune system to fight with cells of thymoma cancer.

Tumor of the anterior mediastinum, which occurs in the epithelial cells of the thymus, is known as thymoma cancer. It is one of the rare leading causes of cancer death, but the appropriate cause of this cancer is not known. The researchers develop advanced immunotherapeutic cancer agents such as monoclonal antibodies, therapeutic vaccines, kinase inhibitor and much more to treat this tumor.

This tumor is classified into five types that involve Type A, Type B1, Type B2, Type B3, and Type C.

Type A thymoma accounts for approximately 4% to 7% of all cases of this tumor, and is also known as spindle cell thymoma or medullar thymoma. 

Type B1 thymoma represents about 9% to 20% of all tumors related to thymoma, and is also known as lymphocytic thymoma, and predominantly cortical thymoma. This type of tumor has a high concentration of lymphocytes, even the cells of the thymus appear in normal condition.

Type B2 thymoma has a high concentration of lymphocytes, are similar to the type B1 thymoma. T cells of the thymus do not appear as normal, and it is also known as polygonal cell thymoma and cortical thymoma. It represents about 20% to 36% of all tumor related to thymoma. 

Type B3 thymoma is also known as squamous thymoma, atypical thymoma, and epithelial thymoma. It represents about 10% to 14% of thymoma tumor. This type of tumor has few lymphocytes, and the cells of the thymus, which is looks like to normal cells.

Type C thymoma is very aggressive. The cells in a thymic carcinoma do not look like normal cells of the thymus. It is also known as thymiccarcinoma; it may start from a pre-existing thymoma that has converted into a more aggressive tumor. This type of thymoma is an advanced form of this tumor.

In immunotherapy, there are Monoclonal Antibodies (MABs) and kinase inhibitors are used for the treatment of this tumor, but there is no single drug has approval by United States Food and Drug Administration (U.S. FDA). However, many drugs, including Cetuximab, Cixutumumab, Saracatinib, and Sunitinib are under various phases such as phase 1, phase 2, phase 3, and phase 4 of the clinical trials for FDA approval.
