Vaccine Based immunotherapy against Gastric carcinoma.

Generally vaccines activate and expand tumor-specific T cells as effector T cells. They may act by increasing already existing immunity, by inducing new immunity, or by anti tumor response alike. Tumor specific T cell can be brought upon by peptides stemmed from tumor-related antigens at the T cell sites. Effective vaccination requires these peptides to be presented by a professional antigen-presenting cell, such as a dendritic cell (DC). Immature dendritic cell with high phagocytic capacity which are gathered in peripheral tissues, are more so localized to tumor growing sites. They take up antigens which are digested into small oligopeptides.  They bound to significant major histocompatibility complex (MHC) class I/II elements for presentation to Cd8+ and to Cd4+ helper cytotoxic T Cells.

Tumor antigen-pulsed DC-based antibodies have been confirmed to activate both Cd8+ and Cd4+ T-cell reactions in patients with advanced malignancies. Although clinical trials utilizing DC-based immunizations in patients with advanced malignancies have encouraged into positive immunologic endpoints. Both protein and peptide targets have been used to attempt to stimulate a specific immune response in gastric cancer.  The experiments based on peptides derived from the tumor associated antigen HER2/neu-derived peptide and MAGE (melanoma associated antigen) which are restricted to MHC class I have been shown to induce cytotoxic T cells against tumors. Gastric cancers typically overexpress HER-2/neu and vaccination using dendritic cells pulsed with HER-2/neu peptide which results in decreased tumor. Therefore, primary goal of a cancer vaccine also part of immunotherapy is not only to prevent disease but to generate an active immune response against an existing cancer.

Immunotherapy says,“Let’s knock down Meningioma".

When one talks about the meningioma, it is the most common variant of intracranial tumour that can be observed. Our brain and spinal cord cover themselves with a thin sheath of membrane. This membraneis comprised of arachnoid cap cells, which gives birth to this disease. Currently, a lot of research has been going on to utilise the benefits of immunotherapy in the treatment of this cancer type.

According to a study conducted in United States in 2010, it was estimated that 1, 70,000 people would be affected by this malignancy. Moreover, 155 deaths were also recorded for the same in small children of 0 to 14 years of age.

If we calculate the incidences of meningioma to other tumours associatedwith central nervous system, it will take 33.8% of the share. As per the age standards, a very small population group is affected by this tumour form.Annually, the increase in the number of cases is found to be 7.8 out of one million cases. Females and people above the age of 85 are more prone to this tumour type. The symptoms can range from headache, nausea and weakness to change in behaviour. Exposure to radiations and hormone based therapy may enhance the risk of developing meningioma. Alterations or deletions in chromosomes, modification of signalling pathways, and growth factors associated with cell growth forms the molecular basis for the progression of this malignancy.

Till now, no FDA approved immunotherapeutic agent has been developed for the treatment of meningioma. But, a number of clinical trials are being conducted for the same. In monoclonal antibody class of agents, Bevacizumab has been put under trials alone as well as in combination with everolimus, an mTOR inhibitor.It is proposed to act against the growth factor receptors and prevent the process of angiogenesis to suppress tumour growth. Non-specific symptoms in primary stage likenausea, fever, vomiting,indigestion and sometimes pain, bloated feeling after eating, loss of appetite, dark colored stools (due to the presence of blood) and a feeling of tiredness (due to anaemia). Late symptoms include jaundice, peritoneal and pleural effusions. Usually, Hepatomegaly occurs in presence of an enlarged left supraclavicular lymphnode (Virchow's sign) is a typical finding.

Immunotherapy is a very viable treatment option for women patients suffering from cervical cancer.

Cervical cancer arise in the cells of the cervix (cervix is the lower part of the uterus in the human female reproductive system). This cancer is one of the leading causes of death across the world. With the availability of newer tools and technologies, it is easy to detect it in starting phase.
Cervical cancer is the seventh most common cancer across the world and it represents 85% of all cancers. About 12,360 new cases and 4,020cases of death due to cervical cancer are estimated in 2014. Etiologic factors of this cancer are human papillomavirus (HPV) infected, cigarette smoking, weakened immune system, having a multiple pregnancy, sexual activity and oral contraceptives.

Cervical cancer is classified into four types, which involve Squamous cell carcinoma (90 %), Adenocarcinoma (10%), while small cell and neuroendocrine carcinoma, and Adenosqaumous carcinomas both are rarely observed.  Squamous cell carcinoma is originated from metaplastic squamous epithelium located between the original and new squamocolumnar junctions, the transformation zone. Adenocarcinoma originates from glandular cells. Adenosqaumous carcinomas are originated from the cell lining of the cervix and gland cells. Small cell and neuroendocrine carcinoma originates from neuroectodermal cells like argyrophilic, neuroendocrine, and melanocytic cells. HPV infection is the most common cause of squamous cell cancer and adenocarcinoma.

There are some immunotherapy agents are approved by the Food and Drug Administration (FDA) that involves Cervarix and Gardasil for the treatment of cervical cancer.

Cervarix is a vaccine which is indicated for prevention of cervical cancer caused by oncogenic HPV 16 and 18, but it does not provide protection against disease due to all HPV types. Some adverse effects are observed, which includes headache, gastrointestinal symptoms and myalgia. Gardasil is also a vaccine to protect against cervical cancers, which is caused by HPV types 16 and 18. Some contraindications are observed like severe allergic reactions to yeast and hypersensitivity.
